PLEASE NOTE THAT YOUR USE OF AND FREE ACCESS TO THE CMT-2 ARE SUBJECT TO THE FOLLOWING TERMS; IF YOU DO NOT AGREE TO ALL OF THE FOLLOWING, YOU MAY NOT USE THE CMT-2.
This website and the Contextual Memory Test (CMT-2) are exclusively intended for use or close supervision by qualified licensed healthcare professionals. This includes Registered Occupational Therapists, Speech and Language Pathologists, or Neuropsychologists with knowledge and skills in the area of cognition, memory, and cognitive rehabilitation or graduate students supervised by a qualified healthcare professional.
The CMT-2 clinical memory assessment is in the pilot phase. Access and use are presently free of charge for use by qualified health providers. Personally identifiable information of clients or healthy volunteers such as name, dates, e-mails, and specific geographic location, is not entered or collected on this website. Because we do not collect personal or "protected health information," the CMT-2 is not subject to the federal Health Insurance Portability and Accountability Act ("HIPAA") or the Children's Online Privacy Protection Rule ("COPPA"). Users are solely responsible for any applicable compliance with local, federal or state laws governing the privacy and security of Personal Information. Identifiable information about your patients or healthy controls should not be shared through our Website.
The CMT-2 does not diagnose memory loss or any clinical condition such as dementia or Alzheimer’s disease. Although the CMT-2 can provide valuable information on memory performance, strategy use and self-awareness that may be useful, along with other assessment information, within a rehabilitation program, results must be interpreted by a qualified licensed health provider, with a minimum of a Master’s degree and training in cognitive impairments and cognitive rehabilitation. The CMT-2 has not been approved by any sanctioning body.
To use the CMT-2 with patients or healthy controls, you will be required to create a user account and password by disclosing the following Personal Information: your name and surname, your email address, and the country where you practice. You must ensure that all information provided by you during the sign-up process is truthful and accurate. Your registration information is stored securely and will never be shared without your permission. Passwords are encrypted with a one-way hash and cannot be decrypted. The CMT-2 website can only be accessed securely via SSL. We will not sell, rent, lease, share, or exchange your name and e-mail with any third-party vendors.
We will send account users emails regarding updates from the Multicontext.net websites such as products, resources, upcoming courses, or information on the Contextual Memory Test (CMT). It is assumed that users who unsubscribe are no longer interested in using the CMT website and therefore such accounts will be deactivated. Any accounts that are inactive for 2 years may be deleted unless requests to maintain the account are received. Users can also request the deletion of accounts at any time by emailing info@multicontext.net
To use the CMT-2 test with patients or healthy controls, you will be required to create a de-identified profile for that person. To do so and, to the extent required by applicable laws, with the prior consent of the individual, you will be required to provide some de-identified information, namely an arbitrary ID number (provided that such number is given randomly such that it is not related to information about the person and cannot be used to identify the person, and does not entail risks of re-identification (e.g., the code or other means of re-identification is not disclosed by you), age, gender and level of education. This ID links the test results to the test report. We will never request, nor should you ever provide to us, any identifiable information (e.g., name, address, etc.) about your clients or healthy volunteers. You are responsible for accurately entering information and for downloading or printing copies of test reports. We cannot guarantee that you will be able to access previous reports nor can we guarantee that you can use the site or the CMT-2 test at times or locations of your choosing.
De-identified demographic information and test results may be used on an aggregated basis to improve the CMT-2, the Website, or Application for our users; for research purposes (for example, to investigate psychometric characteristics of the CMT-2; or increase overall knowledge about memory performance, strategy use and awareness); or for statistical purposes and quality improvement. Users can request deletion of test reports or data at any time, however, once data is analyzed on an aggregate basis, individual data cannot be removed from the aggregated analysis.
The CMT-2 test, website, forms or documents cannot be copied, reproduced, distributed, republished, downloaded, displayed, posted or transmitted in any form or by any means. Permission to modify or otherwise create derivative works of the CMT-2 assessment is not granted. Printed copies, photos and/or videos of the CMT-2 for distribution to others, advertising, publicity, or publications is not permitted.
Translation into different languages or utilization of the assessment for research purposes requires permission by contacting info@multicontext.net.
The CMT-2 assessment, results, and test report are provided as is, without representation as to its fitness for any purpose, and without warranty of any kind, either express or implied, including without limitation the implied warranties of merchantability and fitness for a particular purpose. No guarantees are made with respect to accuracy, completeness, errors, or omissions of results or reports. There is no obligation to provide support, updates, enhancements, or other modifications.
MC CogRehab Resources LLC and authors of the CMT-2 may not be held liable for any claims that may result from the administration or interpretation of this test or any decision made or action taken based on information provided through this assessment. MC CogRehab Resources LLC and authors shall not be liable for any compensatory or non-compensatory damages, including but not limited to special, indirect, incidental, or consequential damages, with respect to any claim arising out of or in connection with the use of this assessment, even if it has been or is hereafter advised of the possibility of such damages.
As noted above, any accounts that are inactive for 2 years may be deleted unless requests to maintain the account are received.
